DESIRED QUALIFICATIONS: BA/BSW in Social Work, Education, Special Education or related field. Experience working with family dynamics and troubled children/youth. Knowledge of education, social service, mental health, other agency service provision to target population. SUPERVISION: District = Bldg. Principal(s). Program = Director of Special Education or Designee, Director/Supervisor County Social Services or designee. Employment/Supervision = Stellher Human Services, Inc LICENSURE: Eligible for licensure by State Board of Social Work and /or Board of Teaching. Licensed by a Professional Board. License must be presented upon request TARGET POPULATION: Learners in grades pre-K through 12 within the geographic boundaries of the Special Education, District and Counties served and who are identified in need of intervention services under MN 125A.50. RESPONSIBILITIES: 1). Collaborate with local human service agencies/providers in the provision of community-based services to targeted learners at the local District site. 2). Assist in the promotion of effective parental / family involvement in the provision of interagency, school-based intervention and academic services. 3). Serve as community liaison between/among agencies in the provision of interagency, school-based academic intervention services to targeted population. 4). Provide direct academic services i.e. social skills-building, anger management and problem-solving individual and small group sessions in math and /or reading to targeted population of learners in the regular education classroom. 5). Work directly with local District Pre-Referral, Student Assistance, Intervention Child Study and/or individual learner program planning Teams. Assist in early intervention. 6). Assist in collection of data foe Project evaluation and for program intervention purposes. 7). Provide Case facilitation as may be appropriate and identified per local agreements with Counties or other agencies. 8). Assist families in accessing services as may be needed from time to time in order to empower families in supporting the learner’s academic success in the classroom. 9).Assist in the prevention of special education and other high cost intervention services. 10). Provide incidental support, services as may be necessary on day-to-day basis in the regular education classroom or as may be assigned and /or approved by the Building Principal.
